Bike Check Out Form

TERMS AND CONDITIONS

Wherever used herein, the term “Equipment” shall include any bike, bike lock and bike helmet checked out from GALLEGOS United, ROX United, CANVAS United, Oak Leaf Productions, South Norte, and AIM Sports Group, herein referred to as “Companies.” The Companies shall not be responsible for any injuries, death, damage, loss, or delay in any means of transportation or by reasons of any event beyond the actual control of the Companies or due to force majeure. Borrowers may use the equipment at their own risk and agree not to hold the Companies responsible for injury or death resulting from accidents. The Company strongly recommends the use of approved helmets whenever using a bicycle.  Helmets are provided with the bike borrowed.  The bicycles provided for use are in satisfactory operating condition and borrower agree to use them at their
own risk. 

RESPONSIBILITY FOR DAMAGE OR LOSS

Borrower agrees he/she will return the equipment in the same good condition as when received, ordinary wear and tear accepted, and to repair and replace lost or stolen, or damaged bicycles or parts or to reimburse the Companies for said equipment. Therefore, regardless of the party at fault, Borrower understands and agrees to be responsible for the theft or damage to said equipment.

Borrower understands and agrees that any sharing with third parties of the borrowed equipment is prohibited and that any such sharing shall immediately cause termination and cancellation of this contract.

BORROWER LIABILITY

Borrower shall assume liability for all damage or loss of and to personal property, accident / injury to other persons related to said equipment.
